    Grooming Release Form

  • Aussie Pet Mobile ATX & Texas State Law requires your pet(s) to have current rabies vaccination for every pet being groomed. By signing this contract, owners verify their pet(s) are current on their Rabies vaccination and that the Rabies certificate is available for inspection. While we adhere to a stringent sanitation procedure for all vans and equipment, we also strongly advise that each pet be vaccinated against Distemper, Hepatitis, Parainfluenza Adenovirus Type 2, Parvovirus, CCV, Leptospirosis, and Bordetella prior to being groomed.

Aussie Pet Mobile ATX has the right to refuse grooming any animal that may be a threat to itself or any person. If we are not able to remove a cat from its carrier or get a muzzle on a dog that may bite, then we may have to refuse service for its own protection and ours.

Shaving a pet often exposes pre-existing skin conditions. If an animal is badly matted, you may find sores and tender skin under those matts. Many times, the skin is so sore that the clippers may cut the skin, especially while taking matts off a cat that has very thin skin. There is no way to avoid this, however slow or careful we are.

Tips are always appreciated but NEVER expected. If a groomer does a particularly good job and/or goes beyond your expectations, then a tip of 10-20% is acceptable.

Hold Harmless Agreement: I hereby release Aussie Pet Mobile ATX from any responsibility of accidents, parasites, medical problems or other problems that may be uncovered while the grooming service is being performed. I hold harmless Aussie Pet Mobile ATX from any damage, loss, or claim arising from any condition of the undersigned pet, either known or unknown.
